Chronic Condition Management

At Rockingham GP, our doctors support patients living with long-term or ongoing health conditions by creating personalised Chronic Disease Management Plans. A chronic condition is one that has lasted — or is expected to last — six months or longer, or one that your GP determines requires continuous care.

Patients who are registered with MyMedicare may also be eligible for Medicare-subsidised allied health services, with up to five visits per calendar year to approved providers such as physiotherapists, podiatrists, dietitians and exercise physiologists.

Each plan is individually designed to improve continuity of care, enhance coordination between your healthcare providers, and support your overall wellbeing.
